Here are a list of reviews that all patients must have if they have the following long term condition:

Diabetic Review: | These are usually done every 12 months if your diabetes is well controlled. We will review every 6 months if adequately controlled, and every 3 months if poorly controlled. For a diabetic review you will need to have the following checks: blood pressure, foot pulses, weight, medication. We would also organise for you to have your bloods checked (including HbA1c), we would ask about your insulin medication or blood glucose levels (if applicable). Once we have your blood results back we would amend medication if needed. You will also need to organise your diabetic retinal screening once a year. |

Dementia Review: | For your dementia review |
Depression Review: | For your depression review we would organise your annual blood tests, check your blood pressure and adjust medication where needed. We would also go through the Patient Health Questionnaire (PHQ-9) and assess the results. |

Mental Health Review: | For your mental health review we would check your blood pressure, organise your monitoring bloods where needed. We would ask you about your alcohol intake, smoking status, illicit drug use, sexual health (if needed). We would also carry out a medication review. |
